Since most every funeral home has a form designed for pre-need, perhaps you could include the most comprehensive form that you can find and automatically include it in every funeral home menu.  If the client would like to change the form, they could just eliminate some lines, or simply change it.  This would eliminate every funeral home client from having to design their own form. (which can be time consuming)
